Information We Collect 

We collect information necessary to provide care, including: 

Identification and contact details 

Billing information (provincial and/or private insurance) 

Health information such as medical history, diagnoses, test results, treatments, allergies, and prescriptions 

How We Use and Share Information 

Your information is used to provide safe and effective care. 

For your care: By seeking care at our clinic, you consent to the use and sharing of relevant information with healthcare professionals involved in your treatment (e.g., specialists, pharmacists, labs, and other providers). 

As required by law: We may disclose information without consent when legally obligated (e.g., public health reporting, provincial billing, court orders). 

To help us provide care efficiently, we kindly ask patients to follow these guidelines: 

Please arrive on time for your scheduled appointment. Late arrivals may need to reschedule. 

If you are feeling unwell during your visit, you may be asked to wear a mask

Patients may be asked to use hand sanitizer upon arrival. 

Please bring only essential personal items and avoid large bags or carts. 

Patients or visitors with symptoms of an infectious illness are asked to wear a medical-grade mask. Masks will be provided if you do not have one. 

Please use hand sanitizer upon entering the clinic. 

Staff experiencing symptoms will either work from home or wear a mask while at the clinic. 

Patients or staff who choose to wear a mask to protect themselves or family members will be respected, and others in close contact may also be asked to wear a mask. 

During severe weather, the clinic may close for staff safety. Appointments will be converted to phone visits. Patients should check weather conditions and call ahead. Urgent conditions should go to urgent care or call 911. 

Becoming a Patient at Flowerstone

If you are interested in joining Flowerstone Family Health Clinic, please review the eligibility requirements below. Our clinic accepts new patients through the BC Health Connect Registry based on location and availability.

  • Local resident of Oceanside (Nanoose Bay to Bowser, and everywhere in between)
  • Have no primary care provider, either a Nurse Practitioner or a Family Physician
  • Families will be attached together
  • Please be aware, patients are required to have a BC Health Care Number (PHN).


Our process/protocol for attaching clients to Flowerstone Family Health Clinic:

New patients will be called on a first-come, first serve basis off of the Health Connect Registry through the HealthLinkBC.ca/health-connect-registry website.

We will not be taking applications by phone or walk-in at this time.

Wait times:
We currently have no estimates for wait times. After opening, each provider will be attaching new clients at a safe and considered rate. You will be formally contacted when a provider can take you on as a new client.
